  • What is Community Outreach?

    At San Antonio Community Hospital, we are committed to building a strong and vibrant community. Through our tradition of caring, we reach out into the community with services and programs to improve the overall health and well-being of those we serve. This is how we provide benefits to our community beyond traditional medical care. Our primary service areas include the cities of Claremont, Chino, Fontana, Montclair, Ontario, Rancho Cucamonga, and Upland.

    In an effort to increase the health and wellness of our community, the Community Outreach department provides a comprehensive variety of health programs and services. Examples of our community health improvement programs include health education seminars and screenings for breast, colon, and prostate cancer; health counseling and screenings at community centers; preventative services such as flu vaccines, diabetes education, and obesity management; and direct medical care for low income, uninsured and underinsured residents. We also conduct a comprehensive triennial community health needs assessment to further evaluate and prioritize community needs.This process culminates in the development of a three-year strategy and implementation plan. In addition to the comprehensive assessment conducted every thre years, secondary data collected through the community health needs assessment is updated annually to continue to identify and meet the needs of the population we serve.

  • Promoting Healthy Communities

    Recognizing the power of collaboration in improving community health status, San Antonio Community Hospital is establishing partnerships with other healthcare providers, public benefit agencies, and community leaders. This commitment has been demonstrated by the effort expended in forging partnerships and leveraging resources to increase access to healthcare services. San Antonio is serving as a voice to support the Healthy Communities movement in informing policy development. Changing the built environment is a key strategy to facilitate healthy eating and physical activity opportunities. One of the best methods to effect such change is to implement policies governing city zoning ordinances at a local level. By working together we can improve the health status of those who live, work, and play in our community.
