
Medical Record Copies
Medical records are confidential and are protected by state and federal
laws. Although the medical record is the property of San Antonio Community
Hospital, the patient has the right to access information contained
in their medical record. You may obtain a copy of the information in
your medical record by signing an authorization to release the information.
There is a fee for personal copies. There
is no fee for sending a copy of your medical record to another healthcare
provider, i.e., physician or hospital, who requests the information
for continuity of your healthcare. To obtain more information on how
to receive a copy of your medical record, please contact Health Information Management
weekdays, from 8:00 a.m. to 4:30 p.m., at (909) 920-4750.

San Antonio Community Hospital retains adult records for 10 years and
children's records for up to 19 years.
Birth and Death Certificates
Copies of birth and death certificates may be obtained by writing to
San Bernardino County's Department of Public Health at the address listed
