

Our mission is to provide healthcare services that comfort and cure.
We understand that emotional and social problems can accompany illness
and our Case Management Team can help patients understand and cope with
illness and treatment.
Our
Case Managers are registered nurses with an extensive clinical background.
They have expertise in communicating with physicians, patients, families,
and insurance companies to ensure a smooth transition from admission
to discharge. Our team is familiar with community resources that can
help patients continue their recovery after they leave the Hospital.
Specially trained social workers assist with difficult issues and decisions
that sometimes must be addressed during an episode of illness. The physician
determines which services are needed and writes an order for post-hospital
care that can include skilled nursing care, acute rehabilitation care,
home nursing, physical therapy, medical equipment, hospice, or other
community services.
Case Management is organized as a unit based model, which allows each nursing station to have a dedicated case manager.
This collaboration and team approach
provides greater continuity of care for the patient, family and physician.

your nurse can page our on call Case Managers.
